Our team at NorthCoast Nurses comprises of experienced and proud Residential Aged Care Registered Nurses, consequently, we are very aware of the challenges/workloads/documentation requirements etc. GP’s face when they have patients/residents living within Residential Aged Care Facilities (RACF’s).
Based on our aged care experience, we have designed unique systems of care we call ‘GPACSS’ (GP Aged Care Support Systems). The fundamental goal of our GPACSS programs are to improve the health and wellbeing of our elderly loved ones residing within Residential Aged Care Facilities whilst simultaneously improving the GP’s experience. Our programs also support RACF’s accreditation compliance regarding standards of care.
Our methods focus on:
- Improving the health and wellbeing of all GP patients/residents within RACF’s, leading to potential reduction in hospital admissions & costs to the health care system,
- Facilitating a wholistic and co-ordinated approach of care whilst simultaneously, promoting RACF compliance and supporting RACF/ACFI funding,
- Reducing GP documentation workloads,
- Reduces the number of frustrating faxes, emails etc. GP’s may receive from RACF’s,
- Promotes GP remuneration for work done,
- Provides GP’s with a Premium Comprehensive Medical Assessment that can be well utilised by all Medical and RACF staff,
- Our invoices are tax deducatable
